EMPLOYEE KILLED BY GANG SAW NOT LOCKED OUT

Investigation abstract

AT 1:45 PM ON MARCH 17, 1987, EMPLOYEE #1, THE OFF-BEARER ON THE GREEN END OF A FRICK 48 INCH COMBINATION GANG SAW RAISED THE LID OVER THE BLADES WITH HIS LEFT HAND TO REMOVE A PIECE OF LOG CAUGHT IN THE SAW. ANOTHER WORKER MISTAKENLY START ED, THEN IMMEDIATELY STOPPED, THE SAW. EMPLOYEE # 1 WAS DRAWN INTO THE MACHINE. HIS RIGHT ARM WAS PARTIALLY SEVERED. FOUR OF EIGHT 48 INCH SAW BLADES PIERCED TH E RIGHT SIDE OF HIS BACK. THE BLADES WERE BACKED UP TO REMOVE EMPLOYEE #1, WHO H AD DIED. THE SWITCH FOR THE ELECTRICAL EQUIPMENT WAS NOT OPEN AND TAGGED, BLOCKE D OR LOCKED OUT BEFORE WORK ON THE SAW WAS PERFORMED.
