Employee injured in 51 ft fall due to defective rigging
Investigation abstract
Employee #1 and coworkers were flying a floating concrete slip form from the fif th to the sixth floor of a building. Employee #1 had descended to the top of the form table and attached the crane hook to a pick point. He then moved to hook u p the second pick point while workers on the fifth floor began to manually roll the form out of the structure. The foreman on the sixth floor noticed an incorre ct connection on the first pick point and instructed Employee #1 to fix it. As E mployee #1 returned to the first pick point, the form became detached from the c rane hook and he fell 51 feet with the form to the concrete base. Employee #1 wa s hospitalized.
