AT APPROXIMATELY 2:55 PM ON MAY 3, 1987, EMPLOYEE #1 WAS INFLATING A SINGLE PIEC E RIM WHEEL WHEN THE WHEEL AND ITS COMPONENTS EXPLODED. EMPLOYEE #1 WAS STRUCK I N THE TOP FRONT PART OF HIS HEAD BY FLYING COMPONENTS OF THE WHEEL RIM ASSEMBLY AND SUFFERED FATAL HEAD INJURIES.
