CONSTRUCTION EMPLOYEE KILLED IN FALL FROM STEEL BEAM

Investigation abstract

DURING THE PAINTING OF A STRUCTURAL STEEL TIERED STRUCTURE, EMPLOYEE #1 NEEDED T O RELOCATE THE BOATSWAIN'S CHAIR RIGGING. HE WAS DOING THIS WHILE STANDING ON AN "I" BEAM WITHOUT HIS SAFETY LANYARD ATTACHED. THE RIGGING CAUGHT ON SOME OBJECT . WHEN THE EMPLOYEE JERKED THE ROPE TO LOOSEN IT HE LOST HIS BALANCE AND FELL 94 FEET TO HIS DEATH.
