EMPLOYEE CRUSHED TO DEATH UNDER OVERTURNED FORKLIFT

Investigation abstract

EMPLOYEE #1 WAS DRIVING A CLARK FORKLIFT ON A GRAVEL AND DIRT COMPACTED SURFACE. USING THE SPINNER-KNOB ON THE STEERING WHEEL, EMPLOYEE #1 TURNED TOO QUICKLY AN D THE FORKLIFT ROLLED OVER. THE DRIVER'S HEAD WAS CRUSHED UNDER THE FORKLIFT.
