Employee killed when struck by whaler thrown from scaffold
Investigation abstract
Employee #1 was 15 ft below an area in which a crew was tearing out wooden forms (whalers) from a bridge under construction. Two workers on scaffolding were rem oving the whalers, which were composed of two 4 1/2 ft long 2 by 10s and weighed approximately 20 to 30 lb each. One of the workers threw down a whaler that str uck Employee #1 on the back of the neck, crushing the vertebrae in his neck, inj uring his brain stem and spinal cord. Employee #1 was killed.
