EMPLOYEE'S HAND CAUGHT IN CONVEYOR CHAIN DRIVE

Investigation abstract

EMPLOYEE #1 WAS FREEING BOARDS WHICH HAD BECOME JAMMED IN THE STACKER CONVEYOR. IN THE PROCESS, HIS HAND WAS KNOCKED INTO A PARTIALLY-GUARDED CHAIN DRIVE AND FI NGERS WERE AMPUTATED.
