Employee's hand amputated by jam bar

Investigation abstract

Employee #1 climbed up onto the #7 plastic injection molding machine to adjust a n air hose that had stuck on the plastic part of the die. While the employee was up on top of the machine, his hand was pulled in front of the jam bar. When the bar closed, his hand was amputated.
