EMPLOYEE #1 WAS CLIMBING INTO A CONFINED AREA TO WORK ON A TURBINE SEAL EXHAUST VALVE THAT COULD NOT BE REACHED THROUGH THE FLOOR HOLE AT THE WORK STATION. THE EMPLOYEE HAD A FATAL HEART ATTACK WHILE TRYING TO REACH THE VALVE.
