EMPLOYEE #1 WAS ADJUSTING THE SPEED OF THE WINDER MACHINE. HIS HAND SLIPPED OFF THE MOTOR, CAUSING HIS RIGHT INDEX FINGER TO BECOME CAUGHT IN THE NIP POINT OF T HE DRIVE PULLEY.
