Employee's hand caught in cotton fabric windup roll
Investigation abstract
Employee #1 was loading an Apex machine with a 12-in.-wide roll of cotton liner ottom cutoff switch. Employee #1 sustained injuries to his left hand that requir ed hospitalization. that was used to separate rubber strips in the manufacture of tire bands. The le ading end was routed around guide bars and placed around the take-up roller. The start limit switch was wired back and the festoon weight that makes the bottom cutoff (limit) switch was secured by the leading end of a rubber flipper. Employ ee #1, who was wearing gloves, had his right foot on the bottom cutoff switch to jog the machine during this loading process.
