Investigation abstract

EMPLOYEE #1 WAS STANDING ON THE GROUND DISASSEMBLING A TWO POINT SUSPENSION SCAF FOLD. A WINDOW SECURITY SCREEN FELL FROM OVERHEAD AND STRUCK HIM IN THE HEAD. EM PLOYEE # 1 WAS THE CREW LEADER AND HAD SEVERAL YEAR'S EXPERIENCE.
