EMPLOYEE'S FINGER CRUSHED IN PRESS POINT OF OPERATION
Investigation abstract
EMPLOYEE #1 WAS OPERATING A PRESS BRAKE THAT WAS NOT EQUIPPED WITH A POINT OF OP ERATION GUARD. HE WAS INEXPERIENCED, ONLY HAVING OPERATED THE MACHINE 21 TIMES P REVIOUSLY. HIS FINGERS WERE IN THE DIE AREA WHEN HE ACCIDENTLY TRIPPED THE THE P RESS BRAKE. HIS FINGER WAS CAUGHT BETWEEN THE PUNCH AND THE DIE.
