EMPLOYEE #1 WAS USING A CUTTING TORCH TO REMOVE THE TOP OF A 55 GALLON DRUM. THE DRUM HAD NOT BEEN CLEANED OR PURGED. THE DRUM EXPLODED, CAUSING A FIRE IN WHICH EMPLOYEE #1 WAS BURNED TO DEATH.
