EMPLOYEE SPRAYED IN EYES BY GAS NOZZLE

Investigation abstract

EMPLOYEE #1 REACHED TO RECEIVE A NOZZLE FROM ANOTHER EMPLOYEE. THE NOZZLE VALVE WAS OPEN, BUT THE PUMP WAS NOT TURNED ON. A THIRD EMPLOYEE TURNED THE PUMP ON, N OT KNOWING THE NOZZLE VALVE WAS OPEN. GASOLINE WAS SPRAYED INTO EMPLOYEE #1'S EY ES.
