Employee killed in fall from roof to concrete surface
Investigation abstract
Employee #1 was preparing to start work on the roof of a single family house. Th e eave-to-ground distance was 25 ft 11 in. and the roof slope was 8:12. Employee #1 was reaching for a roof seat when he lost his footing and fell over the side of the roof to a concrete surface. Employee #1 sustained internal injuries and died.
