Event description
Employee killed when caught between clamshell bucket teeth
Investigation abstract
Employee #1 was standing inside the arms of a 3/4 yard clamshell bucket that had its jaws open. It was resting on its side on a huge metal plate along a 45 degr ee slope. Employee #1 was leaning forward, looking at a piece that had broken of f the counterweight sheave wheel when the clamshell toppled down the slope with Employee #1 inside. When it came to rest approximately 12 ft downslope, the jaws had closed on Employee #1's midsection. His body was pierced by the center toot h, with his head and upper body face down inside the clamshell and his legs pinn ed under it. Employee #1 was killed.
