BURNED BY HOT ROOFING TAR

Investigation abstract

EMPLOYEE #1 HAD POURED A BUCKET OF HOT ROOFING TAR INTO THE HOPPER OF A MIXING M ACHINE. THE TAR WAS TO BE MIXED WITH VERMICULITE FOR USE IN ROOFING. HE TURNED A ND STEPPED OFF THE 18 1/4" HIGH WORK PLATFORM HE HAD BEEN STANDING ON AND PUT HI S LEFT FOOT INTO A 14" HIGH, 5 GALLON BUCKET OF HOT TAR (460 DEGREES). UNKNOWN T O HIM, ANOTHER EMPLOYEE HAD PLACED THE BUCKET OF TAR BEHIND HIM. HE SUFFERED SEC OND AND THIRD DEGREE BURNS TO HIS TOES, FOOT, ANKLE AND LOWER LEG.
