Event description
Employee killed when struck by falling metal stairway
Investigation abstract
Employee #1 was cleaning up cut firebrick fragments on the furnace floor level o f a smelter. At the same time, three workers from another contractor were loweri ng a metal stairway, which was suspended above the area in which Employee #1 was working. The stairway was hooked to a chain fall and to a padeye. The weld on t he padeye broke, dropping the stairway on top of Employee #1 and causing multipl e injuries to his head, neck, chest and back, of which he died. Employee #1 and the workers had not been aware of each others' presence and no warning signs wer e posted.
