Logging truck driver killed by falling logs
Investigation abstract
Employee #1, a logging truck driver, was releasing the binders from the logs on he logs had been poorly loaded onto the truck. his truck and pulling off the wrapper. The load had not been restrained by an un loading machine and two logs fell from the truck, striking and killing him. Empl oyee #1 should have waited for the load to be served by a front end loader befor e removing the wrapper. The sorting yard was narrow and the procedure for removi ng wrappers was badly defined and confusing. During the 2 1/2 weeks prior to thi s fatality, several loads had not been restrained by an unloading machine before the wrapper was taken off.
