Employee injured in fall from stairway opening

Investigation abstract

Employee #1 was working on the seventh floor of a building, using a crowbar to w edge a section of the stairs in place. When the crowbar slipped, he fell approxi mately 65 ft to the ground, sustaining injuries that required hospitalization.
