THE EMPLOYEE PUT HIS HEAD OUTSIDE OF THE TRUCK WHILE IT WAS MOVING. HIS HEAD WAS CRUSHED BETWEEN THE TRUCK BODY AND THE SIDE OF A WAREHOUSE BUILDING.
