Investigation abstract
ON NOVEMBER 14, 1985, AT APPROXIMATELY 2:20 PM, EMPLOYEE #1 FELL FROM A LADDER W HILE SUPERVISING THE WORK OF OTHERS. THE EVIDENCE APPEARS TO SHOW THAT THE EMPLO YEE'S FALL FROM THE LADDER WAS CAUSED BY A HEART ATTACK. THE CAUSE OF DEATH WAS A CARDIAC ARREST.
