Investigation abstract
EMPLOYEE #1 WAS ENGAGED IN WIRE ROPE STRETCHING OPERATIONS. AS HE WALKED OVER A SLACK PART OF THE LINE, THE WIRE GRIPPERS SLIPPED. THE WIRE ROPE BECAME TAUT AND WHIPPED, STRIKING HIM IN THE NECK AND INFLICTING FATAL INJURIES.
