Event description
Employee killed in fall from ladder on cooling tower
Investigation abstract
Employee #1 was working above the deck of a 48 ft high cooling tower. He had one foot on a 10 ft wooden ladder that was leaning against the cooling tower deck a nd being held at its base by a coworker on the deck. His other foot was on a con duit pipe that was approximately 6 ft 3 in. above the deck, which was being remo ved by sections. The coworker felt a gust of wind and the ladder moved toward hi m. Employee #1 fell approximately 54 ft 3 in. to the ground and was killed.
