Investigation abstract
EMPLOYEE #1 WAS ASKED TO OBSERVE THE AUTOMATIC SLIP PLACER ON A ABS PALLETIZER U NIT IN AN ATTEMPT TO DETERMINE THE CAUSE OF A JERKING MOTION. EMPLOYEE #1 LOOKED OVER THE GUARD AND DOWN AT THE CHAIN DRIVE AT THE SAME TIME AS THE MAIN FRAME A ND SLIDER DECK WERE MOVING DOWNWARD TO THE HOME POSITION. HE WAS PINNED BETWEEN THE MAIN FRAME AND THE GUARD AND WAS KILLED.
