EMPLOYEE #1 WAS GREASING GEARS WHILE THE GEARS WERE IN OPERATION. THE EMPLOYEE'S HAND OR ARM BECAME CAUGHT IN AN INGOING NIP POINT. HIS ENTIRE BODY WAS PULLED I NTO THE GEARS AND HE WAS KILLED.
