Event description
Employee dies after falling from roof
Investigation abstract
Employee #1 and his brother were assigned to perform tasks on a roof 28 ft above ground level. They were working approximately 18 in. from the edge of the roof. Employee #1 apparently fell off the roof without his brother being aware that h e had fallen. Employee #1 died ten days later in the hospital of complications r esulting from injuries sustained in the fall. No mechanical equipment was being used on the roof at the time the accident occurred. No safety monitoring system was in use.
