Employee killed when unsecured platform falls

Investigation abstract

On November 4, 1987, Employee #1 was working as a stack crew member. He stepped from the top of an 18 ft high stack of raisins onto an unsecured suspended perso nnel platform. The platform fell, striking his head and killing him.
