Employee killed when struck by falling lumber
Investigation abstract
Employee #1 was operating a Nasco Loadster lift, #DF-16-MI-2679-891, picking up packages of lumber and bringing them to the stacker. He picked up a package of 8 ft long basswood and backed the lift about 6 to 8 ft. He then got off the lift and walked around in front of the load, apparently to check the number. While he was bent over, a package of 12 ft long basswood, weighing 5,000 lb, fell on him and killed him. The wood had been stacked three packages high; each package was double banded with 4 in. by 4 in. spacers in between. The bottom foundation was 6 in. by 8 in. lumber.
