Crane operator killed while hoisting platforms
Investigation abstract
At 7:45 p.m. on April 18, 1989, two employees were moving a 23 ft 6 in. by 11 ft platform to another area to be painted. Employee #1, the crane operator, set th e platform on two boards and leaned the platform against two A-frames. He then u nhooked the slings from the crane. But the platform began to slide out at the bo ttom and the two wheeled A-frames rolled out of the way. Employee #1 fell, strik ing his head against a metal sawhorse. He died of head injuries. Employee #1 sho uld have secured the load before releasing it from the crane. Two previously hoi sted platforms had been secured to A-frames with C-clamps.
