Event description
Employee killed in 22 ft fall from roof
Investigation abstract
At 1:35 p.m. on May 25, 1989, Employee #1 was breaking loose bands around lumber , which was stacked on the edge of a warehouse roof. When Employee #1 broke the second band around the lumber, the stack shifted, struck him in the chest, and k nocked him over the roof's edge. Employee #1 fell 22 ft to the ground level. He sustained serious injuries and died.
