Event description

Employee killed when head strikes roadway after fall

Investigation abstract

While returning from a cleaning assignment, Employee #1 was riding in the open d oorway of a step van. He fell out of the van when it made a shallow turn. Employ ee #1 struck his head on the roadway and suffered a fractured skull. He died.
