CONSTRUCTION WORKER KILLED WHEN STRUCK BY FALLING RISER

Investigation abstract

A CONSTRUCTION CREW WAS HOISTING AND POSITIONING A PREFABRICATED RISER. ONE EMPL OYEE WAS STANDING ON THE RISER OPERATING A CHAIN HOIST. ANOTHER EMPLOYEE (EMPLOY EE #1) WAS STANDING UNDER THE RISER DIRECTING ITS PLACEMENT. THE SUPPORTING BEAM , COMPOSED OF ALUMINUM AND WOOD, SUDDENLY ROLLED, BENT AND BROKE. THIS ALLOWED T HE HOIST RIGGING TO COME LOOSE FROM THE BEAM, WHICH IN TURN CAUSED THE RISER TO FALL. EMPLOYEE #1 WAS STRUCK AND KILLED. THE EMPLOYEE STANDING ON THE RISER (EMP LOYEE #2) FELL WITH THE RISER BUT SUSTAINED ONLY MINOR INJURIES.
