Event description
Employee dies in fall from unsecured plywood patch
Investigation abstract
Employee #1 was a plumber working on the 12th floor of a high-rise construction site, on which flying forms were being used. When lunch time arrived, he left hi s work area on the slab. Work platforms had been placed on the flying forms in a reas where they extended beyond the perimeter of the slabs. Plywood patches supp orted by stringers resting on the flying forms were used in areas where the plat forms did not join due to structural columns. A stripping crew had lowered one o f the tables under a patch and had removed the stringers, leaving the patch supp orted by only one table. Employee #1 stepped onto the patch, which gave way, and he fell to the ground. Employee #1 was killed.
