EMPLOYEE #1 PICKED UP A PIECE OF UNSECURED PLYWOOD THAT WAS USED TO COVER A SKYL IGHT OPENING. APPARENTLY UNAWARE OF THE OPENING, HE STEPPED INTO IT AND FELL 18 FEET TO A CONCRETE FLOOR. HE WAS KILLED.
