ROAD GRADER OPERATOR DROVE INTO CANAL AND DROWNED
Investigation abstract
EMPLOYEE #1 WAS WORKING ALONE IN A SUGAR CANE FIELD, OPERATING A CATERPILLAR ROA D GRADER. HE WAS SUPPOSED TO BE GRADING A ROAD LOCATED ALONGSIDE THE CANAL. AT T HE TIME OF THE ACCIDENT THE GRADER WAS HEADED SOUTH ON A GRASS ROADWAY BESIDE TH E CANAL. HE APPARENTLY LOST CONTROL OF THE GRADER AND DROVE INTO THE CANAL. HE D ROWNED.
