Investigation abstract
Employee #1 was assigned the task of washing a nonskid floor. For unknown reason s, he fell to the floor (from a standing position), struck his head on the floor and suffered a fractured skull. The accident happened at 2 pm on October 20, 19 86. The floor was dry and there were no tripping obstacles.
