Event description

Press operator injures hand when safety device beaks

Investigation abstract

At approximately 8:00 p.m. on January 20, 1987, Employee #1 was operating a Blis s #4 1/2 power press. The dual palm buttons had been taped together with a piece of wood to "chest" the guard. His hand was in the die area when the he clutch s afety device broke, causing the ram to come down on his hand. Employee #1 was in jured and required hospitalization.
