Investigation abstract

EMPLOYEE #1 WAS RIDING ON TOP OF THE SAFETY CAGE ("MONKEY CAGE), ON THE CARRIAGE OF THE STACKER CRANE. THE CRANE IS A MAJOR COMPONENT OF THE AUTOMATIC STORAGE A ND RETREIVAL SYSTEM. HE WAS CRUSHED BETWEEN THE SAFETY CAGE AND THE STEEL CROSS MEMBER WHICH CONNECTS THE DUAL MASTS OF THE STACKER CRANE, 90 FEET ABOVE FLOOR L EVEL.
