Employee killed when struck by falling formwork
Investigation abstract
Employee #1 and two coworkers, all bricklayers, were building a brick wall along 4 formwork. He was killed. the inside edge of the ground floor of a parking deck. Another contractor's cre w was removing ceiling forms three floors above them. A member of that crew was sent down to warn the bricklayers that some pieces of formwork could fall and to keep out of the danger area. Following the warning, the bricklayers stopped wor k. After a while, Employee #1 yelled and asked if the crew was through with thei r work. Because there was no response, he and his coworkers believed the danger was over and went back to work. A few minutes later, Employee #1 was leaning his head over the brick wall when his hard hat was struck by an 8 ft piece of 4 by
