Employee killed when run over by water wagon
Investigation abstract
Employee #1 was using a 1954 Euclid 42 ft 6 in. long, 4,500 gallon water wagon, model #17754, serial #89W6637, to wet down a road under construction. He apparen tly suffered some type of physical seizure and fell 4 ft 4 in. from the operator 's seat to the ground. The water wagon was in motion, and Employee #1's head and upper chest were crushed by the rear wheels. Employee #1 was killed.
