Employee killed in fall down elevator shaft

Investigation abstract

At 2:50 p.m. on March 3, 1986, Employee #1 was using an operational Dover passen ger elevator. While opening the hoistway door from the second floor landing with a special key, he fell through the door and into the elevator hoistway, landing in the pit 33 ft 6 in. below. The elevator car had been stopped on the third fl oor. Employee #1 died of massive head injuries.
