Event description

Employee killed in fall from roof

Investigation abstract

Employee #1 was bolting an air cylinder to a damper on an approximately 43 ft hi gh roof of a wastewater treatment building. The work area was approximately 3 ft from the edge of the roof. Employee #1's wrench slipped from the damper cylinde r rod, causing him to fall to the ground. He was killed. Neither guardrails nor other safeguards were in use at the time of the accident.
