Event description

EMPLOYEE DIED DUE TO FALLING CASING PIPE

Investigation abstract

The employee was standing underneath a suspended 30-ft-long by 7-in diameter cas ing pipe. The pipe elevating attachment failed, opened, and the pipe fell striki ng the employee. He sustained injuries to his neck and back and died.
