AN EMPLOYEE WAS USING A PORTABLE ELECTRIC WELDING MACHINE. THE WELDER WAS CONNEC TED TO ITS ELECTRIC SUPPLY BY MEANS OF AN EXTENSION CORD SET. THE EMPLOYEE WAS E LECTROCUTED. (THE REPORT DID NOT INDICATE HOW.)
