Investigation abstract
EMPLOYEE #1 WAS CLEANING A SPRAY GUN AND SCREEN WITH A CLASS 1B FLAMMABLE LIQUID . APPARENTLY A SPARK IGNITED THE VAPORS, CAUSING A FIRE IN WHICH THE EMPLOYEE WA S KILLED. THE FIRE COULD HAVE BEEN PREVENTED BY ADEQUATE BONDING AND GROUNDING, OR VENTILATION TO REMOVE THE FLAMMABLE VAPORS.
