EMPLOYEE #1 WAS WORKING STRIPPING THE BRIDGE DECK FROM A SCAFFOLD 90 FEET ABOVE GROUND LEVEL. THE SCAFFOLD COLLAPSED, THROWING EMPLOYEE #1 90 FEET TO THE GROUND . HE WAS KILLED.
