EMPLOYEE #1 OPENED THE DOOR TO THE ELEVATOR AND FELL DOWN THE SHAFT TO HIS DEATH . THE ELEVATOR WAS NOT AT THE EMPLOYEE'S LOCATION WHEN HE OPENED THE DOOR.
