Event description

Employee hospitalized after falling through floor opening

Investigation abstract

Employee #1 was cutting down a piece of galvanized conduit for the casting cooli ng chain conveyor system while standing on a 6 ft stepladder. The ladder was nex t to a 6 ft by 3 ft floor hole. He apparently fell 11 ft through the floor hole and landed on his head on the cement floor. Employee #1 was hospitalized.
